Welcome to Cub Scout Pack 476, out of Middletown/Myersville MD!  We are a very active pack with both boys and girls grades K through 5.  In general we meet at the Myersville Fire Station during the cold weather months and meet outdoors (at either Wiles Branch Park in Middletown or Doubs Park in Myersville) during warm weather months.

In addition to an application, we collect the following forms:

• Complete BSA Medical Form and copy of insurance card.  The medical form must be updated annually.  Any adult or child without a current Tetanus and does not wish to immunize should complete a immunization exemption request.
• Please share any resources you may have for the pack with the Family Talent Survey form.
